STUDIO HEALTH TERMS OF SERVICE

Welcome to Studio Health. These Terms of Service ("Terms") govern your use of our telehealth mental health services, website, and any associated offerings. By accessing our services, you agree to these Terms. If you do not agree, please refrain from using our services.
1. Practice Information
Studio Health is a 100% telehealth mental health practice registered in New York State, providing services in accordance with state laws and professional regulations. Our practitioners include licensed mental health professionals and nurse practitioners.
2. Scope of Services
Studio Health offers:
Individual and group mental health counseling
Psychiatric evaluations and medication management
Health coaching and wellness consultations
Community-based mental health initiatives
All services are provided exclusively via telehealth, subject to state regulations.
3. Eligibility
To receive services, clients must:
Be at least 18 years old or have parental/guardian consent for minors
Reside in New York State
Have access to a secure internet connection and a private setting
Complete all required intake forms and agreements
4. Confidentiality & Privacy
We comply with the Health Insurance Portability and Accountability Act (HIPAA) and all applicable New York State privacy laws. Your health information is confidential, except as required by law, such as in cases of:
Suspected abuse or neglect
Imminent harm to self or others
Court orders
5. Telehealth Services
All sessions are conducted virtually. Clients must ensure they have:
A secure and stable internet connection
A private location for sessions
Compatible technology for video conferencing
By using our telehealth services, you acknowledge the potential risks, including data breaches, and agree to the necessary precautions to maintain privacy.
6. Fees & Payment Policies
Payment is due at the time of service unless otherwise agreed.
Studio Health accepts self-pay, and insurance reimbursement may be available for select plans.
Fees for services are subject to change with notice.
A valid payment method must be on file for billing purposes.
7. Cancellation & No-Show Policy
Appointments must be canceled or rescheduled at least 24 hours in advance.
No-show or late cancellations may be subject to a fee.
Repeated missed appointments may result in termination of services.
8. Client Responsibilities
Clients agree to:
Provide accurate and updated personal and medical information.
Engage actively in treatment and follow recommendations.
Ensure a stable internet connection and private setting for sessions.
Treat staff and practitioners with respect.
Adhere to all policies outlined in these Terms.
9. Emergency & Crisis Situations
Studio Health does not provide emergency services. If you are experiencing a crisis, please call 911, visit your nearest emergency room, or contact a crisis hotline such as:

10. Termination of Services
Studio Health reserves the right to terminate services under circumstances including, but not limited to:
Non-compliance with treatment recommendations
Failure to pay for services
Disruptive or abusive behavior
Clients may also choose to discontinue services at any time.
11. Limitation of Liability
Studio Health and its practitioners are not liable for any indirect, incidental, or consequential damages resulting from the use of our services. Services are provided "as is" without warranties of any kind.
12. Changes to Terms of Service
We may update these Terms periodically. Any changes will be posted on our website and will become effective upon posting.
